NSO Certified Marriage Certificate (for married only) 7. PRC License and Board Rating (Duly Certified and Authenticated by the PRC) 8. Civil Service Eligibility (Duly Certified and Authenticated by the CSC) 9. Relevant Trainings and Seminars attended after graduation 10. Certificate of Employment (original) 11.
