2. Curriculum Vitae or Resume with 2x2 recent ID picture (white background)

3. Valid NBI Clearance

4. Diploma and Transcript (School Authenticated)

5. NSO Certified Birth Certificate

6. NSO Certified Marriage Certificate (for married only)

7. PRC License and Board Rating (Duly Certified and Authenticated by the PRC)

8. Civil Service Eligibility (Duly Certified and Authenticated by the CSC)

9. Relevant Trainings and Seminars attended after graduation

10. Certificate of Employment (original)

11. Clearance Certificate from previous employment (if any)

12. Copy of Appointment and Service Record (if government employed)

13. Any BIR Form for Tax Identification Number (TIN)

Interestedand qualified applicants should signify their interest in writing. Attach the following documents to the application letter and send to the address below.
1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) which can be download at www.csc.gov.ph;
2. Performance rating in the last rating period (if applicable);
3. Photocopy of certificate of eligibility/rating/license; and
4. Photocopy of Transcript of Records.
